Pretty Lil Tan LLC — General Liability & Informed Consent Waiver

I voluntarily consent to receive spray tanning services from Pretty Lil Tan LLC and acknowledge that I have had the opportunity to ask questions about the services to be performed.

Skin reaction acknowledgment. I understand spray tanning solutions contain Dihydroxyacetone (DHA). I confirm no known allergies to DHA or sunless tanning ingredients and accept full responsibility for any adverse effects.

Pre-service disclosure. I have disclosed all relevant skin conditions, allergies, medications, and health information. Conditions including pregnancy, eczema, psoriasis, and photosensitivity may affect results.

Color results. Results vary by skin type, preparation, and aftercare. No specific outcome is guaranteed.

Release of liability. I release and hold harmless Pretty Lil Tan LLC, its owners, employees, and agents from any and all claims arising from spray tan services.

Cancellation. Cancellations within 24 hours may incur a $15 fee. No-shows will be charged the full service price.

Photography. I grant Pretty Lil Tan LLC permission to photograph my results for marketing unless I opt out below.

Parental consent — clients under 18

Must be completed by a parent or legal guardian. Clients under 16 are encouraged to have a parent present.

Pretty Lil Tan LLC — Minor Client Parental Consent & Liability Waiver

I am the parent or legal guardian of the minor named below and give full informed consent for my child to receive spray tanning services from Pretty Lil Tan LLC.

Acknowledgment. I understand the service involves application of a DHA spray tanning solution. I confirm my child has no known allergies and no health condition contraindicates this service.

Health disclosure. I have disclosed all relevant medical information, conditions, and medications.

Parental presence. A parent or guardian is encouraged to be present for clients under 16. For clients 16–17, I consent to the service without my direct presence if I cannot attend.

Release of liability. I release and hold harmless Pretty Lil Tan LLC from any claims arising from services provided to my child.

I am the legal parent or guardian and consent voluntarily.
